Animal Facility Access: Contractor

The following steps must be successfully completed for each contractor before they will be authorized to enter any animal facility at Â鶹AV:

Step 1

The contractor's office personnel must request access for all contractors by submitting an Animal Facility Access Request. Email addresses are required for each individual to be cleared. The Â鶹AV employee responsible for the contractor must be listed as PI/ Supervisor.

All contractors must complete and return the to ANIMALOHP@augusta.edu. This tool determines if contractors must be evaluated by employee health.

Step 3

All contractors must complete Vivarium Hazards Training. This training requires about 30 minutes.

Step 4

Laboratory Animal Services training is required, once all prior steps (1-3) are completed and clearance is granted by Occupational Health/ Employee Health. The LAS Training Coordinator will contact individuals to schedule this training. The Â鶹AV Department requesting access will be notified of completed contractor clearance.
