Family Support Forms
The Office of New Student and Family Transitions is committed to supporting families throughout your student's university experience. We've created two specialized forms to ensure your questions and concerns reach the appropriate staff members quickly and efficiently.
Which Form Should I Use?
Student-Specific Questions Form
This form is designed for parents and family members who need assistance with specific concerns about their individual student that may require access to educational records or detailed follow-up with campus departments.
General Questions Form
We're here to support you and your student throughout their 麻豆AV journey! This form is designed for general questions about university policies, services, procedures, and resources that don't require access to specific student information.
Response Timeline
- General questions: 1-2 business days
- Student-specific concerns: 2-3 business days
- Urgent matters receive priority attention
Privacy & FERPA Compliance
All submissions are handled in accordance with federal privacy regulations. For student-specific concerns, detailed information will be communicated directly with your student, while you'll receive confirmation that follow-up has been initiated.
